Plenary A:
Data Makes a Difference: Measuring Diabetes and Obesity in the Indian Health System

Continuous improvements in individual and population health status require ongoing attention to quality. Indian Health Service is incorporating cost, efficiency and quality transparency into our health care delivery models. This plenary presentation will describe these forces and how they affect delivery of health care in the present and the future for chronic conditions like diabetes and obesity.

  1. Discuss how electronic information requires changes in the health care delivery systems.
  2. Discuss ways to increase collaboration between program staff and information technology specialists.
  3. Discuss future need for data and how data can help improve the care and health status of  AI/AN patients and populations.

 

Plenary B:
Current Knowledge of Prevention of Diabetes and Obesity

The session will present a summary of what we currently know about prevention of diabetes and obesity. Discussion of pertinent outcomes and measures to track success in diabetes and obesity programs will be presented. A summary of major prevention studies and translation of results of the DPP into a community style model will be discussed.

  1. Describe what is known about prevention of diabetes and obesity.
  2. Describe various data and outcome measures to track success in programs.
  3. Discuss the process used to translate the results of the DPP into the YMCA community style model.
  4. Describe the adaptation to the DPP intervention to enhance dissemination and sustainability.
  5. Discuss the issues related to data, evaluation and cost of translation projects.

Plenary C:
Current Knowledge of Prevention and Treatment of Childhood Overweight

This plenary will provide an overview of the current knowledge of prevention and treatment of childhood overweight and summarize relevant childhood overweight research. Emphasis will be on pertinent outcomes and measures to track success in childhood obesity programs. A description of the “Fit Kid” and “Fit Teen” weight management programs at UC Davis Children’s Hospital will be included.

  1. Describe what is known about the prevention and treatment of childhood overweight.
  2. Describe various data and outcome measures to track success in childhood overweight programs.
  3. Describe the process used to translate the known 'best evidence' on childhood overweight into the UC Davis Fit Kids and Fit Teens programs.
  4. Describe the data collection and evaluation of the UC Davis Fit Kids and Fit Teens programs.

 

Plenary D:
The Cost Effectiveness of Preventing Type 2 Diabetes

I will describe the costs associated with diabetes and its complications and review and discuss the effectiveness and cost-effectiveness of various intervention strategies to prevent type 2 diabetes in adults with impaired glucose tolerance.

Training and Technology in the Electronic Work Environment

Providing on-going training to employees to meet the demands of their organizations is challenging given the realities of limited dollars, time and travel Post 911. The advances in web technology will help meet the demands for training, as well as provide training that fits the learning style of the new gen Y learners. Increased use of electronically generated data will require on-going and innovative way to enhance skills of workers at all levels in an organization. Examples of how the Division of Diabetes and Office of Information Technology will provide training in the future will be presented.

  1. Describe unique learning and development issues associated with the different generations in the workplace
  2. Discuss the future of providing training and disseminating information using on-demand education and multi-media modalities.
  3. Describe the data needs related to training.
  4. Discuss the future plans for providing training to users of the Resource and Patient Management System.

Achieving Best Health Care Outcomes for the Population

This plenary presentation will discuss key concepts from the Institute for Healthcare Improvement (IHI) for transforming the health care delivery system and measuring improvement. The partnership between IHI and IHS chronic care initiative will be described by a panel of IHS leaders.

  1. Describe the IHI model for improving health care delivery systems and the Care Model.
  2. Describe the state of the art in data and measures for quality improvement.
  3. Describe the how the partnership between IHS and IHI brings value to the quality improvement efforts of both organizations.
